Madison Direct Primary Care ("we," "us," "our," or "the Practice") respects your privacy. This Privacy Policy explains how we collect, use, and protect information gathered through our website, www.madisondirectprimarycare.com (the "Site"). It does not apply to protected health information (PHI) created or received in the course of providing medical care to patients — that information is governed separately by our HIPAA Notice of Privacy Practices, available upon request and provided to all patients at the start of care.

If you become a patient of Madison Direct Primary Care, the collection, use, and protection of your protected health information (PHI) is governed by the Health Insurance Portability and Accountability Act (HIPAA) and described in our Notice of Privacy Practices, which is provided separately and available upon request. Please do not include PHI in website contact forms or unsecured email.

7. SMS / Text Message Communications

Mobile opt-in, SMS consent, and phone numbers collected for SMS communication purposes will not be shared with any third party or affiliates for marketing purposes.
